How this works: check a box once you have considered it and made your choice — even if your choice is “not for me,” “not yet,” or “not applicable.” This is a record of deliberate decisions, not a to-do list you must finish. There are no wrong answers, and every box can be checked. When you check an item, today’s date fills in automatically — click the date to change it to the day you actually signed a document or made the decision, if you know it.

Each box you check is a step from “I should” to “I did.” As these fill in, the question the book asked can be answered with a quiet, confident yes: the people you love would know what to do.

Marking every item does not mean your planning is complete or correct. A document protects you only if it is signed, witnessed, and where required notarized exactly as the law demands, and a trust that is never funded can fail when it is needed most. Only a licensed attorney can confirm what fits your situation and whether your documents are valid and enforceable.

This is a simplified checklist, and it leaves out a great deal. It sorts a broad subject into a few general areas so it is easy to use. It does not address many matters that can be important to a plan, including, for example, tax planning, creditor protection and spendthrift provisions, special-needs and Medicaid planning, business interests, blended-family and non-citizen-spouse issues, and much more. The absence of a topic here does not mean it does not matter to you. A plan that is right for one person can be wrong for another.

Do not act without a lawyer’s review. Please do not create, sign, change, or rely on any estate planning document, or decide not to, based on this ledger. Estate planning documents have legal consequences that depend on your specific facts and on current law. Have a licensed attorney in your state review your circumstances and any documents before you rely on them.
